Will identify touchpoints within school environments to identify connections for families; help schools understand potential partners and resources in their communities, and how to make connections with them. |

District and community perspectives are captured and incorporated to understand the current context, key players and their roles, existing resources and trends, and opportunities for the future. |

This is a web-based application used by community-based organizations to safely and securely make and receive referrals for families they work with who need services beyond what is provided by the district. This ensures that staff are supported with the resources needed to make vital connections and thus improve student safety and ability to learn. |

This plan focuses on essential, high-impact strategies delivered in a condensed, accessible format. Training includes behavior management, instructional support techniques, communication strategies, and clarity around roles and expectations. |
| Transition Support and Planning | Support schools in the identification of needs and partnerships between community providers and schools for children and families transitioning from early childhood to primary school settings. |
